Skill 5: Reading Weight

Skill level-Easy

The nursing care plan will tell you how often a patients height and weight should be taken. You can also ask your charge nurse at any time to see if its needed.





Supplies:

  • Scale
  • Recording Sheet
  • Chair


How to perform the skill:

  1. The patient is in the chair.
  2. Wash Hands
  3. Provide Privacy
  4. ask them if they are dizzy if they are not dizzy ask them to stand (Hold onto one arm, and assist if needed)
  5. Ask them if their dizzy again
  6. Assist with walking to scale (Keep asking if they are ok)
  7. Pause in front of scale.
  8. Zero out the scale
  9. Ask if dizzy
  10. Have them step up onto the scale
  11. Ask if dizzy
  12. Weigh the patient then ask them to step down and assist back to chair
  13. Ask if their dizzy.
  14. Record weight on sheet.
  15. Wash hands, give call light, un-provide for privacy


Notes:

  • You must clear the scale before weighing and after this could be a privacy problem if someone else see’s the weight.
  • The trick is to always ask if their dizzy as this could become a serious problem if they are.
  • As a personal suggestion, you should hand sanitize your hands after leaving the room any time.
